Application Forms
First time applicants, children under 16 applying or renewing, or renewing a lost or damaged passport, use form DS-11
Renewing an adult passport by mail us form DS-82

Required Documents
Proof of Citizenship (Birth Certificate, Immigration Documents, etc)
Identification Documents (State issued picture ID, Drivers’ License, etc)
Proof of Relationship for children applications under 16 years. If parents can’t be present for the application, a notarized form DS-3053 is required.
Photo
You will need two 2″x2″ color photos. There are many specific requirements and your application can be delayed for an incorrect photo, so it is best to get one taken for you. Many post offices and drug stores will take official passport photos for you and you can often get them from the passport office as well, but these will be more expensive. Remember, no glasses or headwear are allowed.
Submit Application
Form DS-11 must be submitted in person at an acceptance center or Passport Agency. Otherwise mail in according to the instructions on the form.

Processing Time
Routine processing times 6-8 weeks
Expedited processing times 2-3 weeks. If you need a passport in less than 6-8 weeks, follow the procedures below for expedition.

- Make an appointment at a Passport Agency or center that processes same day applications.

Extra information
If you think you may travel extensively, you want to order the larger passport book. The price is the same but it offers 52 pages vs the standard 28 page book.
Don’t forget to sign your passport or it will not be valid.
